Negligence Pattern Unveiled: Amid the investigation into the death of an elderly woman at the Columbine Chateau assisted living facility in Pueblo West, concerns have been raised about the caretaker, Tyshae Carbajal, who is now facing charges of Negligently Causing Death and Negligently Causing Serious Bodily Injury to an At-Risk Adult. Prior to her employment at Columbine Chateau, Carbajal worked at the Boone Guest Home, where she was fired for caretaker neglect, as confirmed by a former supervisor. The family of the deceased woman, Marie Bayer, and Carbajal’s former employers are questioning how she was hired again given her previous record of negligence.

Disturbing Past Incident at Boone Guest Home: Before her tenure at Columbine Chateau, Carbajal was involved in a concerning incident at Boone Guest Home, an overnight assisted living facility for individuals with developmental disabilities. Thomas Moore, the assistant director at Boone Guest Home, revealed a disturbing event in September 2022 when police and fire had to be involved due to Carbajal’s behavior. Body camera footage and a police report detailed the chaotic scene, with a nonverbal resident found alone in Carbajal’s car, the home in disarray, and evidence suggesting potential substance abuse. Despite being terminated and reported to Adult Protective Services, Carbajal later secured employment at Columbine Chateau, raising questions about the effectiveness of background checks and the hiring process within the care industry.
